The website provides information about Diagnex for doctors, hospitals and professional partners and allows eligible professionals to express interest in working with Diagnex. Website content is informational and does not replace independent clinical, legal, tax or professional advice.
Submitting an empanelment or onboarding application starts a review process only. It does not create an empanelment, clinical privilege, referral arrangement, employment relationship, patient-volume commitment, commercial entitlement or binding agreement. Any active relationship is subject to separate review, credential verification and formally agreed terms.
The treating doctor retains responsibility for clinical judgement, diagnosis, prescription and medication decisions. Diagnex Care Managers and operational teams work within defined roles and do not replace or override the treating doctor.
A service or pathway described on this website is not a guarantee of availability for a particular patient, locality or date. Relevant scope, operating readiness, professional availability and locality are confirmed before a care pathway is accepted. Future or developing capabilities are not represented as live unless deployment and verification evidence exists.
Diagnex seeks to distinguish current operating evidence from future plans. Public proof points on this site should be read in their stated context; for example, the approved Home Hemodialysis proof point is 250+ sessions delivered, and the healthcare legacy dates to 1995.
You are responsible for ensuring that information and credential documents you submit about yourself are accurate and that you are permitted to provide them. Do not upload patient records, prescriptions or unnecessary patient-identifying information through the professional application forms.
